The aim of this study is to analyze and adapt an Assessment Questionnaire Portfolio of students of the Primary Health Care area, for use by students and teachers in a higher education institution (HEI) and detect the perception of undergraduate student on the definition of portfolio, its advantages and disadvantages for use in higher education. The Assessment Questionnaire Student Portfolio in the Area of Primary Health Care, was submitted to the judges of the FAIMER-Brasil Institute Program and teachers of HEI through the application of an instrument containing Likert Scale 4 and 6 points and its content has been revised and adapted to the reality of the higher education institution studied. In the discovery of students' perceptions of the 2nd year of Medicine and Nursing on the subject portfolio used the technique of structured group interview, recorded by audio. The score of the Likert scale covered a ''concept'' “GOOD” and ''VERY GOOD” with a level of reliability from 0.8 to 0.9 (Cronbach's alpha) among judges and teachers. Among the key findings in the reports submitted by students and teachers stressed the necessity of conducting theoretical training on the topic'' portfolio'' and the possibility of a standardized Assessment Questionnaire Student Portfolio in the Area of Primary Health.
