This essay presents a brief historical analysis of medical education in Brazil throughout its academic and pedagogical evolution. In order to meet social demands and the contemporary concepts of health and sickness, new models of education have emerged in recent decades, as options for the training of medical professionals, both nationally and internationally. In this essay, we focus on the following topics: a) higher education and medical schools in Brazil, b) curricular architecture in medical education: challenges and possibilities; and c) reflections on curricular changes and active methodologies in the profile of the graduate physician. Regarding the changes in medical professional training, we come to the conclusion that changes in curricular architectures do not appear to be sufficient, in themselves, to change the profile of professionals. In Brazil, there are very few studies about physicians trained by the different methods and curricula, and the discussion about the transformative efficiency of the different curricular architectures remains unresolved.
